The objective of this initiative was to improve the quality of care for residents of long-term care facilities who have diabetes by improving glycemic control, increasing comprehensive diabetes management, reducing fragmented care, and empowering patient-care teams to educate patients and families regarding this disease. The primary objective of improving glycemic control was demonstrated by a significant 18% reduction in the percentage of residents experiencing hypoglycemia following the intervention, as well as positive changes in 3 additional measures of patient health.

The objective of this initiative was to improve the quality of care for residents of long-term care facilities who have diabetes by improving glycemic control, increasing comprehensive diabetes management, reducing fragmented care, and empowering patient-care teams to educate patients and families regarding this disease. The primary objective of improving glycemic control was demonstrated by a significant 18% reduction in the percentage of residents experiencing hypoglycemia following the intervention, as well as positive changes in 3 additional measures of patient health.

This initiative also seeks to improve the linkage to specialty care for patients who have a positive HCV antibody test.
