The objective of this initiative was to improve the quality of care for residents of long-term care facilities who have diabetes by improving glycemic control, increasing comprehensive diabetes management, reducing fragmented care, and empowering patient-care teams to educate patients and families regarding this disease. The primary objective of improving glycemic control was demonstrated by a significant 18% reduction in the percentage of residents experiencing hypoglycemia following the intervention, as well as positive changes in 3 additional measures of patient health.

Managing Diabetes Among Patients in Long-Term Care Facilities: A Quality Improvement InitiativeEvaluating the Challenges and Impact of Birth Cohort Screening for Hepatitis C in the Primary Care Clinic
The objective of this initiative was to improve the quality of care for residents of long-term care facilities who have diabetes by improving glycemic control, increasing comprehensive diabetes management, reducing fragmented care, and empowering patient-care teams to educate patients and families regarding this disease. The primary objective of improving glycemic control was demonstrated by a significant 18% reduction in the percentage of residents experiencing hypoglycemia following the intervention, as well as positive changes in 3 additional measures of patient health.

This QI initiative, which involves 25 clinics across the US, expands the reach and continues the success of previous QI efforts designed to support clinicians as they change clinical practice patterns to better align HCV testing practices with updated guidelines from the Centers for Disease Control and Prevention and the US Preventive Services Task Force. This initiative also seeks to improve the linkage to specialty care for patients who have a positive HCV antibody test.
